A Chronic Special Needs Plan, or Medicare Advantage C-SNP, is a type of health coverage designed for people living with certain chronic conditions and needs. It pays to understand what a C-SNP is, who qualifies, and how the application process works. For some people, a C-SNP may be the best choice given their situation, so it should be weighed against the other available health insurance options in order to make an informed decision.
C-SNPs are one category of SNP, and enrollment is restricted to people determined to have a particular special need or chronic condition (or conditions), as defined by the Center for Medicare and Medicaid Services (CMS) and named in its guidelines.

People with a chronic condition, or a mix of chronic conditions, usually need their condition and treatment tracked and coordinated across several care areas, such as their primary care doctor, specialists, and different facilities. A C-SNP can offer stronger coverage for those who require this kind of care for their chronic condition(s).
To enroll in any Medicare Advantage plan, you must already be signed up for Medicare Part A and Part B. Additional requirements vary by the particular plan. For instance, qualifying for a C-SNP largely comes down to which chronic condition you have and whether it appears in the CMS guidelines. At present, those guidelines cover 15 conditions, which are:
For a few of these conditions, eligibility may be narrowed to specific disorders. As an example, a person with an autoimmune disorder who has polyarteritis nodosa, polymyalgia rheumatica, polymyositis, rheumatoid arthritis, or systemic lupus erythematosus would not currently qualify for a C-SNP. Anyone with one of the 15 main conditions should look further into whether their particular disorder is excluded.
Applying for a C-SNP may require you to supply proof of the condition that qualifies you. You would also need to apply for the specific plan you wish to join. Enrollment is generally open during an initial enrollment period, general enrollment, or the Fall open enrollment.
